Care Coordinator (PRN)
Job Description
The purpose of this position is to provide discharge planning services, initial assessment and support to patients hospitalized on the inpatient behavioral health services. The inpatient social worker reports to the Inpatient Medical Director for Behavioral Health. They work closely and in collaboration with other inpatient staff including psychiatrists, mid-level providers, therapists, nurses, program specialists and aids, and clerical support staff.

Qualifications
Bachelor’s degree in human services field from an accredited educational program. 2+ years working with individuals with chronic mental illness. Ability to work cooperatively with the multi-disciplinary team. Excellent written and verbal communication skills. Ability to verbally and physically de-escalate a client utilizing BMC trained programs. Ability to complete electronic charting and documentation
